Every recommendation carries its evidence tier. Tier 1 is large-trial validated. Tier 3 is mechanistic and provisional. Patients see the tier; the tier governs the decision.
Measure. Intervene. Re-measure at 90 days. Re-measure at 180. The loop is not a follow-up; it is the unit of care.
