ApoB: the quietest number that runs the show

On why ApoB, not LDL-C, anchors the metabolic protocol — and where the evidence holds.


The single most evidence-supported lever in cardiometabolic medicine is also the one most patients have never had measured. ApoB counts atherogenic particles, not the cholesterol inside them. The literature on its causal relationship to atherosclerotic cardiovascular disease is, by any reasonable standard, settled.

That settledness is rare in longevity medicine. We name it explicitly here because the rest of the field rarely does.

This post does not recommend an intervention. It defines a number, names the literature behind it, and notes the population in whom that literature is strongest. The clinical decision belongs in clinic — between physician and patient, with the full panel in hand.
